SAM V1:Referrals

The referrals table lists the entities or individuals that referred the student to disability services as well as the entities or individuals to which the student was referred by disability services. A referral record exists for each entity, so if the student was referred by 1 entity and referred to 2 entities, then the one student parent record will have 3 child referral records.

Fields

The fields of the referrals table are:

Field	Description
Date Created	Date record was created.
Date Modified	Date record was last updated.
Referral ID#	Unique ID# of the record.
Record Owner	User who owns the record.
Last Modified By	User who last modified the record.
Address	Address of the referral entity or individual. Format is address, city, state, and zip code.
Email	Email of the referral entity or individual.
Name	Name of referral entity or individual.
Notes	Notes regarding the referral. The notes field is a cumulative field and date stamped with the date, time and user adding the notes.
Phone	Phone number of the referral entity or individual.
Referral	Entity or individual who referred student or to whom the student was referred: Faculty, Counselor, Vocational Rehabilitation, Self, etc. Referral is required.
Student ID	The student the referral is related to. Student ID is required.
Type	Type of referral: Referred To or Referred By. Type is required.
