The case notes table records the cases or interactions between students and disability service providers. A case notes record exists for each meeting or contact, so if the student has met with disability services 3 times, then the one student parent record will have 3 child case notes records.
Fields
The fields of the case notes table are:
Field | Description |

Date Created | Date record was created. |
Date Modified | Date record was last updated. |
Case Note ID# | Unique ID# of the record. |
Record Owner | User who owns the record. |
Last Modified By | User who last modified the record. |
Duration | Calculated field of elapsed duration by subtracting start date and time from end date and time. |
End Date | The date the meeting or contact ended. End date is required. The default value is the current date. |
End Time | The time the meeting or contact ended. End time is required. |
Notes | Notes regarding the case. The notes field is a cumulative field and date stamped with the date, time and user adding the notes. |
Purpose | Purpose of the meeting or contact: Intake, Counseling, Training, Support, etc. Purpose is required. |
Service Provider | The service provider who conducted the meeting or contact. The default value is the current user. Service provider is required. |
Start Date | The date the meeting or contact started. Start date is required. The default value is the current date. |
Start Time | The time the meeting or contact started. Start time is required. |
Student ID | The student the case note is related to. Student ID is required. |
Type | Type of meeting or contact: Office Visit, Phone, Email, Instant Message, etc. Type is required. |
